Group Registration

If you want to register as a Group with COSA's International Service Organization, so that people in your area can be referred to you for either attending or starting a COSA meeting, fill out the registration form below.

To be eligible for membership in COSA-ISO, your group may not have any other affiliation.

If you are representing a COSA meeting that is either new or needs to update its information, then please consult with the other members of your group about what to put on the form.

If you are an individual in an area where there are no meetings, and if you are willing to commit to starting a meeting, then this form will allow you to register as a COSA group. Then if anyone else in your area is looking for support, he or she can be referred to you for starting a new group. If you would simply like to know about others in your area, with whom you can discuss the possibility of starting a group, you are encouraged to register as an Individual.

You are eligible for COSA membership if your life has been affected by compulsive sexual behavior.

Please print out this form, complete it, and send it to:

  1. We need a mailing address where we can send ISO information that pertains to the business concerns of COSA as a whole. This address will not be given out to anyone; it will only be used for ISO business mailings.

    Group's business mailing address:

    __________________________________________

    __________________________________________

    __________________________________________

  2. The ISO of COSA maintains a contact list so that our volunteers can link individuals searching for groups in their area with existing groups or with other individuals committed to starting a new COSA group. The volunteers only give out this information to individuals in need of a meeting, and they respect your anonymity.

Your city and state will be added to the contact list. Please tell us what other information we can provide to newcomers:

City & state (if you are in a smaller town, state directionally which city your meeting location is closest to. For example; Katy, TX -- west of Houston):

    __________________________________________

    Meeting day: ______________________

    Meeting time: _______ am / _______ pm

Group type (from question #5): ___________________________________

    Group name: ___________________________________________

Meeting location (facility, street address, city, state, room/other):

    First name: _____________________________ Phone: ___________________

    Group's phone line: _____________________________

    Fax: _____________________________

    Group mailing address:

    __________________________________________

    __________________________________________

    __________________________________________

  3. May we give out your contact information via e-mail?

    Yes ___ No ___

This means that if one individual using his or her personal e-mail account writes to an ISO of COSA volunteer, then that volunteer can reply via e-mail with your group's contact information.

  4. Your city and state will be listed on COSA-ISO's internet homepage.

Is your group willing to have your contact information published here?

    Yes ___ No ___

This information is available to the general public as well as COSA groups, so your group may prefer to list a post office Box instead of someone's home address. It is not advisable to include a name and home phone number, but that is up to you/your group.

Please fill in any contact information that you want to include on the homepage:

    __________________________________________

    __________________________________________

    __________________________________________

    __________________________________________

    __________________________________________

  5. Group type (indicate all that apply): open / closed / mixed / women / men / lesbian / gay

    __________________________________________

There are many definitions for "open" and "closed" groups. This form's reference to "open" should be taken in the most liberal sense and refers literally to meetings open to anyone interested in finding out about COSA; they need not have admitted that their lives have been affected by compulsive sexual behavior, and there need not be a twelfth-step process for entering the meeting.

  6. Does your group belong to an Intergroup? Which one? _____________________________

If not, does your group communicate with other groups? Which ones?

    __________________________________________

  7. Estimated date of group formation: _____________________________

Estimated number of core group members: _____________________________

  8. Does your group have any other affiliations? _____________________________

COSA-ISO does not wish to receive information about individuals without their knowledge and permission. If you are thinking about filling out one of these forms for a friend or family member, please do not do so. We encourage you to let the other person know about COSA, and ask if they would like a copy of the form to fill out for themselves. If they request assistance, then, of course, feel free to provide it with the other person's active participation.

COSA encourages potential new members to seek help for themselves when they are ready.
